Issue

Issue: On a certain aerial device, the wrong bolts may have used to mount the pedestal to the sub-base. As a result, the bolts may fail. If multiple bolts fail, the pedestal and sub-base could separate from the vehicle. Safety Risk: If the pedestal and sub-base separate from the vehicle, there may be a risk of injury. Corrective Actions: Altec will notify the owner by mail and advise them to perform a visual inspection of the bolts that attach the pedestal to the sub-base. If any grade 5 bolts are installed, an inspection should be completed on a daily basis until the fasteners have been replaced. Within 30 days of the initial inspection, the grade 5 fasteners should be replaced with grade 8 bolts.
