Product description

This recall involves Streamlight 4AA flashlights manufactured before May 2007 with LED module numbers 68211, 68222, and 68223. The module number appears on the UL and cUL panel on the reverse side.

The flashlight uses 4 AA-size alkaline or carbon zinc batteries, and is approved for use in the following locations (as listed on the UL and cUL panel):

  • Class I, Division 1, Groups C&D (US only)
  • Class I, Division 2, Groups A, B, C, D
  • Class II, Division 2, Groups F & G
  • Class III Hazardous (Classified)

Hazard identified

Several units have become hot upon initial use, causing smoke, charring, and melting of components. This may result in a burn to the user, or pose a fire or explosion hazard.

There have been 4 reported incidents in the United States, with no resulting damage or injuries.

The Electrical Safety Authority and Health Canada have not received any reports of injuries or incidents in Canada.

Number sold

Approximately 48,924 units were sold in Canada. Approximately 3,911 were sold in Ontario.

Time period sold

The recalled products were sold from July 2002 to April 2007.

Place of origin

Manufactured in the United States.

What you should do

Consumers should immediately stop using the recalled flashlights.

Recalled units purchased before May 2007 can be returned to Streamlight for replacement. Unpackaged product affected by this recall can be identified by the date code (in the format MM YY) that appears on the flat area on the side of the lightblub.
