This voluntary recall is due to the potential for breaches to the sterile barrier identified during packaging testing, which creates a potential sterility concern. The specific potential failures are one or several of the following:
- inadequate sealing of sterile barrier packaging
- shipping box does not adequately protect device during transportation
- tube twist-off cap failure
