Product description
This recall involves the HiLo "Evolving" chair, made by Age Design Kids Inc. The chair is intended for children aged 6 months to 6 years.
The chair is adjustable to two heights, has a removable tray with a locking system, and a wooden base. The back two legs of the chair have wheels.
Only chairs with a date of manufacture before April 1, 2010 are included. The date of manufacture can be found under the rotation mechanism on each side of the chair.
Hazard identified
If the child is not correctly strapped into the chair with the safety belts, or if the safety belts are not adjusted properly at the waist, the child can reach the chair's removable tray with his or her mouth. There may be enough space between the upper and lower parts of the removable tray that may lead to the child getting their bottom teeth stuck if they try to bite or chew on the tray.
If the child's teeth are stuck, it is very important not to pull on the removable tray as this action could cause the child to loose his or her teeth.
When using the chair, caregivers should ensure that the child is properly held in by the safety belts and is always supervised. Consumers can contact Age Design Kids Inc. by telephone if there are any questions regarding safe usage of the chair.
Health Canada and Age Design Kids Inc. have received 2 reports in Canada of children's teeth becoming entrapped or wedged, with 1 report of loss of teeth.

What you should do
Consumers should immediately examine the lower part of the inside edge of the removable tray, to see whether the gap between the two parts poses a hazard for a child. Consumers are advised to contact Age Design Kids Inc. to receive a free repair kit that will cover the gap between the upper and lower parts of the tray.
Consumers are asked not to return the chair to retailers.
