Issue
The unit package seal failed and it compromised sterility. Although one defect was detected in the field, no adverse events have occurred as a result of this incident. Inspection of inventory on-hand determined that there may be around 1% of this product (p/n 20400161S, lot #185198) with opened seals and if used could result in patient infections.
