Reason
Since May 2006, eight separation events involving aluminum cylinder systems have been reported to western/Scott Fetzer Company involving Western Model RG3000 and RG4000 series valve integrated pressure regulators (VIPRS) manufactured for use on aluminum cylinders. Of the eight events, five of them have been reported since July 2011.
