Fluorescent ballast and light fixtures with installed fluorescent ballasts
Product description
This recall involves ballast models DBAG-13P2C and SY-213HPF with a date code of "082003" and light fixture model LUNAR-2x13-120-HPF, with two plug-in 2-pin 13W compact twin tube fluorescent lamps.
The ballasts were sold separately to be installed into fluorescent light fixtures. Also, an unknown number of the same ballasts were used in production of the light fixtures mentioned above and such ballasts are built-in inside the light fixtures. The listed ballasts and the light fixtures are being recalled.
Hazard identified
High ambient temperatures might cause the ballast to overheat, creating a possible fire hazard.
Symban Lighting has received two reported incidents of overheating. No injuries have been reported.
Health Canada has not received any reports of incidents or injuries related to the use of these ballast and light fixtures.
Number sold
Approximately 1,300 units of the affected ballasts were sold in Canada. The number of recalled light fixtures is unknown.
Time period sold
The affected products were sold from 2003 to 2010.
Place of origin
Manufactured in Taiwan and Canada.

What you should do
Consumers should immediately stop using the ballasts and light fixtures and contact a licensed electrical contractor to disconnect affected units and return them to Symban Lighting for a free replacement. An application for inspection from the Electrical Safety Authority is required.
