Product description

This recall involves Black Diamond Index Ascenders which are aluminum handles that allow a climber to ascend a rope.

The following products are included in this recall:

The affected products were manufactured between December 22, 2014 and February 25, 2016 and display manufacturing codes 4356 to 6015.  The four-digit manufacturing code can be found inside the ascender next to the toothed cam.

Hazard identified

Some rivets in the product may not have been correctly installed. Product with incorrectly installed rivets can fail during use, and could result in the user being seriously or fatally injured as a result of a fall.

Neither Health Canada nor Black Diamond has received any reports of consumer incidents or injuries to Canadians related to the use of this product.

No reports of injury have been received in the United States.

Number sold

Approximately 300 units were sold in Canada, and approximately 4,000 units were distributed in the United States.

Time period sold

The recalled products were sold from December 22, 2014 to March 9, 2016.

Place of origin

Manufactured in the United States.

What you should do

Consumers should immediately stop using the product and contact Black Diamond to arrange for an inspection by Black Diamond or by place-of-purchase personnel, or directions for self-inspection. Black Diamond will replace any improperly manufactured units.
