Product description
The recalled play yards are model number 76361-VT sold under the "Sesame Beginnings" by Kolcraft brand. The model number is printed on a white sticker located on one of the feet of the play yard.
Other play yards named in the US CPSC recall were not sold in Canada.

Kolcraft Recalls Play Yards After the Death of a 10-Month-Old Child
Hazard identified
In the US, there has been 1 reported death which occurred when the child was placed in the playpen with the change table attachment still in place; this is a misuse of the product. The restraint system of the change table consists a strap that loops underneath the attachment. The child strangled on this restraint strap. The playpen includes instructions which state that the child is not to be placed in the playpen when the attachment is affixed to the product.
In Canada, there have been no reported deaths or injuries.

1,000 infant playpens are being recalled.
