Product description

This recall involves the attachment handle on a 20 foot Wire Rope Dyna-Lock®Self-Retracting Lanyard (SRL). It is a fast-acting fall arrester that limits free-fall distance, deceleration distance and fall arrest forces while allowing user freedom of movement.

The models of the Dyna-Lock® lanyard included in this recall are identified with the following Mine Safety Appliances (MSA) Company part numbers: 506619, 506615, 10017931, 10006446, 10006447 and 100017933.

As a precaution, this recall is extended to consumers who purchased units manufactured or had their unit last serviced from April 2005 through June 2008. The date of manufacture and date of last service are identified on a label affixed to the side of the unit.

Hazard identified

The two screws securing the handle were not swaged (flattened) as intended by MSA's housing/handle supplier. The swaging process prevents the screws, which are installed from inside the housing, from unthreading. If a handle screw unthreads due to vibration or other means, the handle could detach from the housing and cause the SRL to separate from the anchorage resulting in loss of fall protection and posing a risk of injuries to consumers.

Health Canada has not received reports of incidents or injuries to Canadians related to the use of these lanyards.

The number of the recalled Dyna-Lock® lanyards sold in Canada is not known.

Time period sold

The recalled Dyna-Lock® lanyards were sold from April 2005 to June 2008.

Consumers should immediately stop using the lanyards with the unswaged screws.

Consumers may return the affected product to the manufacturer, Mine Safety Appliances Company, for repair free of charge.
