2.5mm Lifeport Adapters and Patient Circuit Kits

Bunnell has received reports of 2.5 mm Lifeport adapters with deformations at the patient end tip. These deformations may result in a narrowing of the effective internal diameter of the Lifeport. An evaluation of returned adapters has confirmed the presence of deformations. Bunnell incorporated is voluntarily recalling a limited number of 2.5mm Lifeport adapters (lot number 22042320) for use with the lifepulse high frequency ventilator. The affected 2.5mm Lifeport adapters were packaged in these products: catalog #9025 (2.5 mm Lifeport adapters 10-pack) and catalog #938 (patient circuit kits).
